How do I report a claim?

PLEASE REPORT YOUR CLAIMS AS SOON AS POSSIBLE. CHECK THE LIST BELOW TO DETERMINE WHO TO CONTACT TO REPORT YOUR CLAIM, OR CONTACT YOUR AGENT.

When filing a claim, please be prepared to provide the following:

  1. Your name, address and phone number
  2. Insurance policy number
  3. Date and time of your loss
  4. Brief description of what happened
  5. Insurance and contact information for others involved
